When a change may be needed
A change may be required after shifting residence, transfer/posting or movement to another CGHS-covered city. First confirm whether your beneficiary category requires a formal dispensary/Wellness Centre change.
Official change-of-dispensary form
The current VMMC & Safdarjung Hospital forms repository, an official MoHFW institution, continues to list an 'Application for change of dispensary (in triplicate)'. This confirms that a formal change process remains part of official CGHS administration.
Pensioners shifting city
MoHFW has documented online transfer of the same CGHS card for pensioner beneficiaries who shift residence to another CGHS city. The same card can be used after transfer rather than issuing a completely unrelated beneficiary identity.
Using another Wellness Centre
Government material also records that online connectivity has enabled beneficiaries to avail CGHS facilities from Wellness Centres across the country. However, your registered Wellness Centre/city details should still be kept correct for administration and local services.
What to keep ready
Keep the current CGHS card/e-card, beneficiary ID, new residential address proof and any service/transfer document relevant to your category.
Serving employee
A serving employee should follow the current departmental/CGHS route where office certification or administrative action is required.
After the change
Check the current e-card/index card or beneficiary profile and confirm that the Wellness Centre/city details are correct. Keep the acknowledgement until the change is visible.
If the request does not update
Raise the issue with the concerned CGHS unit using beneficiary ID, old and requested Wellness Centre/city, request date and acknowledgement. Avoid submitting repeated duplicate requests without referring to the first one.
