Start with the service point
For a local Wellness Centre, medicine or beneficiary-record issue, first approach the concerned CGHS unit/CMO in charge or the authority handling the service. Keep the acknowledgement or written response.
For reimbursement claims
Quote the Claim ID/claim number, beneficiary ID, submission date and exact deficiency or delay. The current CGHS system and myCGHS provide online claim-status checking, so include the visible status when escalating.
Hospital problem
Keep the referral, hospital name, date, treatment, estimate/bill, denial or cash-demand evidence and the exact issue. Do not send a general complaint without identifying the service/treatment involved.
Use CPGRAMS for unresolved public grievances
MoHFW's grievance material identifies CPGRAMS as an online public-grievance route. When escalating, describe previous action taken and attach only the documents needed to show the unresolved issue.
What to write
State beneficiary details safely, CGHS ID/reference, date, location/service, exact problem, previous complaint number, response received and the specific relief requested.
Current Ministry grievance officer
MoHFW publishes a current Public Grievance Officer and contact details on its live Grievance Redressal page. Officer names and contact details can change. Use the current MoHFW grievance page for the latest details.
Do not create duplicate complaints immediately
If a complaint already has an active reference, follow that reference and add the missing evidence where the system allows. Multiple duplicate complaints can make tracking harder.
No universal resolution promise
Resolution time depends on the authority and issue. Use the acknowledgement, system status and applicable grievance instructions.
