First identify the exact error
Check whether the problem is in the principal cardholder name, dependent details, date of birth, relationship, mobile number, beneficiary ID, entitlement or another field.
Use the current beneficiary record
Download/view the current CGHS card or index card through the official CGHS/myCGHS system. NIC states that myCGHS allows beneficiaries to download CGHS cards and index cards.
Prepare supporting evidence
Keep identity proof and the service/pension/family/dependency document that proves the correct entry. Use only the documents relevant to the field being corrected.
Family-member changes
Addition, deletion or correction of a dependent can require proof of relationship and continued eligibility/dependency under CGHS rules. A family relationship alone does not automatically prove current dependency eligibility.
Mobile-number problem
If login, OTP or app access is affected, first determine whether the mobile number in the beneficiary record is wrong or whether the issue is only device/app related. Ask for correction of the actual record only when needed.
Serving employees
Where the card/beneficiary record is linked to the serving employee's office records, the department may need to certify or initiate the correction.
Pensioners
Pensioners should use the current CGHS pensioner/card correction route and keep the acknowledgement and supporting documents.
After correction
Download or view the updated e-card/index card and check every changed field. Do not assume the correction is complete merely because a request shows closed.
