Before acting: CGHS procedures differ by beneficiary category, treatment and current orders. Verify the latest requirement for the individual case.

When referral is normally used

MoHFW's 2025–26 Annual Report states that beneficiaries are referred by CGHS Wellness Centres to Government hospitals or specialists of empanelled hospitals for expert consultation, and may then be referred onward for indoor treatment when required.

Specialist OPD at empanelled private hospitals

The same current Annual Report states that OPD specialist consultation at empanelled private hospitals is permitted on referral by a CGHS Medical Officer/CMO.

Beneficiaries aged 70 years and above

MoHFW's 2025–26 Annual Report states that beneficiaries aged 70 years and above are entitled to direct specialist consultation at empanelled hospitals. Before attending, verify the current operational requirements and hospital empanelment.

Referral validity

A Ministry order dated 10 December 2018 states that referral for specialist consultation at an empanelled private hospital is valid for 30 days, for up to three consultations in the same hospital, and may cover consultation with up to three different specialists during a single visit. Because CGHS instructions can later change, check the current rule when using an older referral.

Investigations and listed treatment

The 2018 order states that advice for listed investigations is valid for 30 days and advice for listed treatment procedures for three months. Unlisted procedures/investigations can require competent-authority approval depending on beneficiary category and circumstances.

What to carry

Keep the current CGHS card/e-card, referral or prescription, identity details and relevant reports. If the hospital asks for additional approval, request the exact reason and rule/document needed.

After specialist consultation

The 2018 simplification instructions say the beneficiary should report back to the concerned Wellness Centre for endorsement/supply of medicines and handling of investigations as applicable.

Avoid a common mistake

Do not assume that one referral automatically authorises every later procedure or a different hospital. Match the referral, specialty, hospital, investigation/procedure and validity period.
