Official application exists

The current official VMMC & Safdarjung Hospital forms page lists an 'Application for addition/deletion of name in C.G.H.S card (in triplicate)'. Use the current form/process applicable to your office or CGHS unit.

Before adding a dependant

Confirm that the person falls within the applicable CGHS definition of family and satisfies the current dependency conditions. Relationship alone is not always sufficient.

Documents

The official new-card form asks for age proof for listed family members and proof that dependants stay with the principal beneficiary. Depending on the dependant, additional proof such as disability or other eligibility documentation may be required.

Dependency changes must be reported

The official CGHS new-card declaration requires the applicant to intimate CGHS immediately if dependency criteria of an included family member change.

When deletion may be needed

A family member should not remain on the card when eligibility/dependency has ceased under the applicable CGHS rules. Other changes can arise from death, marriage, separate entitlement or correction of an erroneous inclusion.

Serving employees

For serving employees, family changes may require verification or forwarding by the sponsoring department/office. Keep office-certified records where required.

Check the updated card

After processing, download/view the current e-card or beneficiary record. Confirm the person's name, relationship, date of birth and beneficiary details rather than assuming closure means the record is correct.

If addition/deletion is rejected

Ask for the exact eligibility/document deficiency. Correct that point and resubmit with the acknowledgement/reference rather than creating a different version of the same request.
