Current contribution slabs
The current official CGHS membership schedule lists monthly contribution of ₹250 for Levels 1–5, ₹450 for Level 6, ₹650 for Levels 7–11 and ₹1,000 for Level 12 and above.
Annual contribution for pensioners
The corresponding annual amounts shown are ₹3,000, ₹5,400, ₹7,800 and ₹12,000 respectively.
Lifetime pensioner card
The official schedule shows lifetime contribution of ₹30,000, ₹54,000, ₹78,000 and ₹1,20,000 respectively. These are 120 months, or 10 years, of the monthly contribution.
Which pay level is used
MoHFW's subscription order states that pensioner/family-pensioner contribution is linked to the contribution applicable with reference to the pay level connected to the post held at retirement/death, subject to the detailed order.
Existing lifetime-card holders
The subscription order states that pensioners who already obtained a lifetime CGHS card after paying the prescribed lump sum do not have to pay an additional amount merely because contribution rates are later revised.
Annual contributors
Pensioners paying annually must follow the current rate applicable to them. The Government order contains special instructions on how revised rates apply until the period required for lifetime coverage is completed.
Before paying
Confirm the beneficiary category, applicable level, amount and current payment method from the official CGHS process. Do not pay using an old forwarded challan or stale bank instruction.
Keep payment proof
Save the challan/receipt and application acknowledgement. It may be required during card generation, renewal or correction.
